Transitions of Care Toolkit
The Transitions of Care Toolkit is an APhA member-only resource for pharmacists, pharmacy residents, student pharmacists, pharmacy technicians, and interprofessional health care team members involved in providing transitions of care (TOC) services. The document provides an evidence-based summary of TOC resources in one central location. Use and disseminate this toolkit to help improve the quality of patient-centered care, to reduce health care costs, and to optimize medication therapy during TOC.

Applying the Pharmacists' Patient Care Process to Care Transitions Services is an APhA member-only resource for pharmacists, student pharmacists, and others involved in providing pharmacy-based care transitions services. The document focuses on applying the Joint Commission of Pharmacy Practitioners (JCPP) Pharmacists’ Patient Care Process (PPCP) to the provision of care transitions services. Transitions of Care Case Examples Resource
The Transitions of Care Case Examples were designed as a members-only resource for pharmacists and student pharmacists to improve transitions of care (TOC) services across a multitude of health care settings. The document addresses pharmacists’, student pharmacists’, and pharmacy technicians’ roles in transitions of care services within a community hospital, an outpatient health system pharmacy, an ambulatory care clinic, a long-term care facility, and a community pharmacy.
